ST2 Prognosis Management in Heart Failure

Two views of the AFIAS ST2 test strip

ST2 is regarded as an important biomarker, particularly in prognosis management in heart failure. ST2 is a protein known as the interleukin-33 (IL-33) receptor, which plays a role in the cardiovascular stress response. Serum levels of ST2 are used as an indicator of processes such as cardiac loading and fibrosis (tissue hardening) in patients with heart failure.

ST2 and Heart Failure

  • Biological Role: ST2 takes part in inflammatory response and fibrosis processes. In heart failure, ST2 levels rise, which reflects the damage and stress occurring in the heart muscle.
  • Prognostic Value: High ST2 levels are associated with a worse prognosis in patients with heart failure. Using ST2 together with other biomarkers (for example BNP or NT-proBNP) allows the severity of the disease and the response to treatment to be assessed more accurately.

Areas of Use of ST2

  • Risk Stratification: ST2 levels are used to predict outcomes such as the risk of death and the likelihood of hospitalisation in patients with heart failure. High ST2 levels mean a higher risk, which may require treatment plans to be reviewed.
  • Monitoring Response to Treatment: ST2 levels can also be used to monitor the response to treatment. For example, if ST2 levels remain high despite treatment, this may indicate that the treatment strategy needs to be reviewed.
  • Combination with Other Biomarkers: ST2 is usually used together with other heart failure biomarkers such as BNP or NT-proBNP. This combination helps to develop a more accurate prognosis and treatment strategy.

Advantages of ST2

  • Stable Biomarker: The biological half-life of ST2 is quite long and its biological variability is low. This makes measurement of ST2 more reliable.
  • Independent of Renal Function: ST2 levels are largely independent of kidney function. This makes ST2 a valuable biomarker, particularly in heart failure patients with renal failure.

Clinical Applications and Management

  • ST2 Monitoring: Regular monitoring of ST2 levels in patients with heart failure plays an important role in treatment planning. High-risk patients may require more aggressive treatment and monitoring strategies.
  • Treatment Modification: Treatment can be modified according to ST2 levels. For example, high ST2 levels may require intensification of treatment or the introduction of additional treatment strategies.

Conclusion

ST2 is an important tool in the prognosis management of patients with heart failure. High ST2 levels are regarded as a factor that increases the risk of worsening of the disease and the mortality rate. When used together with other prognostic tools, this biomarker offers a more effective approach to the management of heart failure.
